Genetic Analysis of a Large Family with Migraine, Vertigo, and Motion Sickness

Background: Migraine is a common disorder most typically presenting as headache and often associated with vertigo and motion sickness. It is a genetically complex condition with multiple genes ultimately contributing to the predisposition and development of this episodic neurological disorder. We identified a large American family of 29 individuals of which 17 members suffered from at least one of these disorders, migraine, vertigo, or motion sickness. Many of these individuals suffered from several simultaneously. We hypothesized that vertigo and motion sickness may involve genes that are independent to those directly contributing to migraine susceptibility. Methods: Genome-wide linkage analysis performed using 400 microsatellite repeat markers spaced at 10 cM throughout the genome. The members of this family were phenotyped for each condition, migraine, vertigo, and motion sickness and analyzed separately. Statistical analysis was performed using two-point and multipoint linkage analysis employing a number of models including autosomal recessive or dominant patterns of inheritance with high and low genetic penetrance. Results: We identified a novel locus for migraine, 9q13-q22 (maximum two-point logarithm of odds [LOD] score-2.51). In addition, there are suggestive LOD scores that localize to different chromosomes for each phenotype; vertigo (chromosome 18, LOD score of 1.82) and motion sickness (chromosome 4, LOD score of 2.09). Conclusions: Our analysis supports our hypothesis that the migraine-associated vertigo and motion sickness may involve distinct susceptibility genes.

Background
Reaction time (RT) variability is one of the strongest findings to emerge in cognitive-experimental research of attention deficit hyperactivity disorder (ADHD). We set out to confirm the association between ADHD and slow and variable RTs and investigate the degree to which RT performance improves under fast event rate and incentives. Using a group familial correlation approach, we tested the hypothesis that there are shared familial effects on RT performance and ADHD.
MethodA total of 144 ADHD combined-type probands, 125 siblings of the ADHD probands and 60 control participants, ages 6–18, performed a four-choice RT task with baseline and fast-incentive conditions.
ResultsADHD was associated with slow and variable RTs, and with greater improvement in speed and RT variability from baseline to fast-incentive condition. RT performance showed shared familial influences with ADHD. Under the assumption that the familial effects represent genetic influences, the proportion of the phenotypic correlation due to shared familial influences was estimated as 60–70%.
ConclusionsThe data are inconsistent with models that consider RT variability as reflecting a stable cognitive deficit in ADHD, but instead emphasize the extent to which energetic or motivational factors can have a greater effect on RT performance in ADHD. The findings support the role of RT variability as an endophenotype mediating the link between genes and ADHD.

Summary
Introduction
Botulinum toxin (BoNT), the most potent biologic toxin, is a highly effective therapeutic tool in a variety of neurologic and other disorders. In minute doses it has the ability to block Ach release and, therefore, has been exploited to relieve disease symptoms associated with muscle hyperactivity. Intramuscular injection of BoNT produces a chemodenervertion at the neuromuscular junction that results in reversible partial paralysis. While used primarily for conditions characterized by abnormal, excessive, or inappropriate muscle contractions, BoNT is also increasingly used in the management of a variety of ophthalmologic, urologic, gastrointestinal, orthopedic, cosmetic and dermatologic disorders.
As the use of BoNT continues to increase, the antigenicity of BoNT and development of immunoresistance secondary to blocking antibodies has continued to be a pressing concern. A certain percentage of patients receiving repeated injections develop blocking antibodies (immunoresistance) against BoNT (BoNT-Abs) causing them to be completely resistant to the effects of subsequent BoNT injections. This is termed secondary resistance. Primary resistance refers to lack of response to initial BoNT treatment, which is extremely rare, and may be due to pre-existing BoNT-Abs, possibly as a result of prior immunization against botulism. The frequency of neutralizing or blocking antibodies (immunoresistance) against BoNT is not known. This lack of information is partly due to a paucity of well-designed epidemiological studies utilizing appropriate assays to determine the frequency of blocking antibodies in a prospectively followed population of BoNT-treated patients.
